Intracavernosal Injection (ICI)

An intracavernosal injection (ICI) is a treatment method used primarily for erectile dysfunction (ED). It involves injecting medication directly into the corpus cavernosum, the spongy tissue inside the penis that fills with blood during an erection.


Here’s how it works:

  • The medication—often alprostadil, or a combination like Trimix (alprostadil, papaverine, and phentolamine)—relaxes blood vessels and increases blood flow, triggering an erection within 5 to 20 minutes.
  • It’s typically used when oral medications like Viagra or Cialis aren’t effective or suitable.
  • The injection is done with a very fine needle, usually at the 2 or 10 o’clock position along the shaft (never the head), and alternating sides is important to avoid tissue damage.
  • Erections from ICI can last 30–60 minutes, but if it lasts longer than 2 hours, it could lead to priapism, a medical emergency.
